- Who it helps:Adults age 60 and older with a primary mental illness diagnosis who need ongoing specialized community support.
- How to start:Referrals go through Home Care Case Management or Continuing Care Access.
- What to expect:Support may include medical monitoring, medication support, therapies, geriatric psychiatry, recreation, and practical daily-living assistance.
- Clinic hours:Monday to Friday 8:00 am - 4:00 pm
- Cost:Monthly fee applies. Reductions or waivers may be available for some clients.
- Eligibility:Age 60+ with a primary mental illness diagnosis; excludes people whose primary diagnosis is dementia with psychiatric symptoms.
